clean dos extender executable format (Developers)

I already have that, and it relies on HX or equivalent.

I thought I was learning from others right now.

Regardless, just because I already have something
working, relying on HX, doesn't mean that that is
the cleanest implementation.

All I did was copy Microsoft's Win32.

I'd like to independently derive something from
first principles, and using the benefit of hindsight.
If that is actually possible.
