The best pure typesetting systems are TeX and its derivatives

PCroff is a derivative of the Unix roff print util. Includes source (DeSmet C).

GNU enscript converts ASCII to PostScript, HTML, RTF, ANSI, and overstrikes (but only PS has full support).
