Note I'm using an older version of Wine - newer versions now behave differently and as above with newer versions they appear to require DOSBox vs having basic DOS support internally.

As I now understand it with newer Wine versions, yes. There older versions had some limited v86 mode support internally. On newer Wine, there is apparently also now a config setting to enable/disable early Windows app support (as per links above) due apparently to issues with the way DOS/NE stub handling works.
