What is VGAMTRR? If it is something to set the memory region A0000-AFFFF to WC, this can be done with current VESAMTRR. It's not recommended to do so, however, because 4bpp modes won't work anymore with WC being set.

It depends on the definition. HDPMI32 itself runs in ring 0 (this is mandatory to be able to switch to real/v86-mode), but it runs its clients in ring 3.
